  I used the mask that gives the drill almost exclusively. Dont worry about taking too much time, just make sure you let the enemies see you first (this will add to the "boldness" score at the end of the level) then lure them back to another room, preferably one by one. Knock them down (make sure they are laying flat on the floor) then do the drill kill on the floor. This alone will get you 2200 points per kill.
  
  On a side note- I was having a TON of trouble on chapter 10. This chapter and only two others were ones that i needed for the trophy. As an alternative method, back up your save data to a usb drive, delete and reinstall the game but dont install the patch, play a chapter you havent a+ yet and once you do it should pop. It did for me anyway. So now I have the trophy but i didnt have to do chapter 10 and one other chapter.

  Whilst combos are important, you can also pop this trophy using the drill. Equip the Carl mask which gives you a drill at the start. Take your time, knock down enemies and then execute with the drill.
  
  Remember to use doors and you can also throw the drill to knock enemies down. Firing guns to lure enemies is also a useful trick.
  
  The drill execution leaves you vulnerable for a good chunk of time so be patient. The drill is useless on big enemies and dogs.

  Unfortunately he doesn't use the drill method in all of his videos, I've linked the one I got the idea from. I wish I had the facility to record as I have managed to get a+ on all levels with the drill.
