This is for getting 50 kills with the pistol that you start the game with. If you don't have this by the time you beat the game then just boot the game up on easy on the first level and exclusively get kills with this pistol and you should get this soon enough.
Also you should pause and save game after getting it so that the game keeps the medal points.
