To achieve this ending you have to elect Duncas as the high king, and then reach the ending without an alliance amongst the factions. When facing the final choice between joining Constantin and killing him, make sure you choose to kill him. Once all of these steps are completed the trophy should pop once the final cutscene monologue starts.
