To unlock the Natural instincts trophy in DayZ, you will need to hunt a deer and gut it.
You can generally find deers in the forests of Chernarus. Sneak up on the deer without spooking them and shoot one of them. Once killed, use your knife to gut it. Once done, the trophy will unlock.
