To unlock the Madman trophy in World War Z, you will need to complete 100 games in PvE. Each time you complete an episode you will gain 1% progress towards unlocking the trophy. It doesn't matter if you are online with other people or playing offline with bots, and the difficulty doesn't matter either.
Simply finish an episode for progress to count. Failing or quitting out before you finish the episode won't count.
