First things first. Get your Hardcore run done first. That's the worst part. Just worry about completing the game and not about completion scores.
Note: You'll only get eternal voyage if Duke and Damir are NOT in the group at the end of the game.
Once that's done (you're likely to have gotten the bad ending), complete the game on Reader difficulty collecting all collectibles apart from any in Taiga, and completing the
trophies which will in turn unlock
at the end of the game. On this run also avoid killing the Blind Ones as this helps with Karma points and nets you a seperate trophy (especially if you didn't do it in Ranger Hardcore).
The Guide trophy in Metro Exodus worth 51 pointsPass the Research Facility without killing a Blind One on the DEAD CITY level.
Also, make sure you keep Alyosha unhared by doing
The Forest child trophy in Metro Exodus worth 35 pointsComplete the TAIGA level without attacking anyone or getting noticed.
You can also tie these trophies in to your Forest Child run:
The Decommunization trophy in Metro Exodus worth 28 pointsDestroy the biggest statue in front of the children's camp on the TAIGA level. The 5 o'clock trophy in Metro Exodus worth 24 pointsTake part in the Admiral's tea party on the TAIGA level.
A note on difficulties, don't forget
can't be unlocked if you're on Ranger Hardcore and must be done on your collectibles playthrough.
Go back and finish the Taiga collectibles which unlocks both collectible trophies for "Old World Pictures" and the diary trophy. There's a third collectible category for Suit Upgrades. This will be finished by Caspian and you'll get the achievement as soon as you've collected the final one.
A note on suit upgrades:
There are none in Yamantau, Taiga and Dead City (these locations do have diary and postcard collectibles though).
You may still have some kill trophies left. If you have the
trophy left to do. Reload Dead City, and only use the Helsing through the entire section through the warzone. Save when you get to the point where Miller says "The battle ended here" and then reload when all of the Nosalis are dead.

being their last trophy. If this is the case, go back to Volga and every so often jump in the water and run along the shore close to the water to get them a bit dirtier. I noticed weapons also pick up dirt if you smash them in to a bandits face like a melee weapon. DO NOT swap weapons out or pick up new parts when trying for this trophy.
Go to an workbench often and check your weapons status. Wait till it shows 40 or so resources to clean and then back out the bench amd quick save, clean and back out, reload the quicksave file and keep doing this until it unlocks. (Weapons DO wear out quicker on Ranger Hardcore, but they also become more useless, meaning, you'll be relying on stealth).
seems to be one people struggle with too. Playing through Yamantau and doing the process for 90 kills:
starts you off well. Before then, if you kill every Bandit on Volga, that's 40 kills easy. Bandit camps on Taiga are also fair game (as long as no Pioneers or Pirates die if you want Alyosha to keep his legs). And Caspian is fair game everywhere as long as you refrain from putting bullet holes in to the slaves.
Use your easy difficulty playthrough to worry about the following:
The Kaleidoscope trophy in Metro Exodus worth 29 pointsKill 3 enemies using sniper scope while wearing the Gas mask and using the Night Vision goggles.
The Last Breath trophy in Metro Exodus worth 25 pointsAfter running out of filters in a hazard zone, craft a new one while suffocating.
