You need to go to the director's office and pick up the "nondescript keycard". It is on the top floor of the hospital area where Erich Soders is being operated on.
Disposable scramblers are quite useful to get to this area without much hassle, you can unlock this item by getting Mastery Level 15 in Colorado.
Now go down to the morgue area, and near where the heart is being held, or where the dead yellow biker is, there is a body refrigerator which you can open with the keycard.
Inside will be Agent Smith who is a recurring character in the Hitman series!
Trophy will unlock when you free him by unlocking the fridge.
