This achievement is obtainable the day after finding the second mirror. Do not fast travel. Go to the alter where you first saw the cat to start the quest. Talk to the little girl twice. Once it's added to your notebook to find the cat turn around and go back up the hill and go down the steps on your left. At the bottom to the right will be the cat.
