You can get this at Film 3 Chapter 5. As you play the level you continue to go up the ship's masts, each time to a higher level. There's at least three levels (there might be a fourth on the final mast) on different masts. Wait til you go to the absolute highest mast (near the end of the stage) at that point jump off, and the trophy will pop.

It's very easy to do, though you may miss it the first time, you CAN earn it in the story mode, or free play mode.

As with the other solution here it is on Story 3 the last level, called The Maelstrom.

Has to be done from the highest mast, which you do not reach till the last stage when fighting Davy Jones and he is on his last heart.
Do not attack him yet but run along away from the screen and jump off the mast into the sea.

NOTE: I did this with the Fall Rescue extra turned on.
