To start a cross chain, you must get an enemy into the 'Vulnerable' state. This will often happen after 'Breaking' an enemy, executing a parry, or just dealing a lot of damage quickly.
Once an enemy is vulnerable, there is a seemingly random chance for one of your comrades to call out to you to initiate the cross-chain. When this happens, a yellow box will pop-up on the screen that says 'Press Triangle to warp-strike and initiate a cross-chain'. A full-hit cross-chain requires you to complete the QTE mini-game that starts once you do the warp-strike. It takes ten successful hits (pressing Circle when indicated) to finish the combo.

If you start playing the game from scratch after the update, this should come pretty naturally while doing hunts and side-quests around the world. I found that enemies with higher armor & HP were much easier to work with for this since you can make them vulnerable without killing them. Once you start the combo, you won't kill the enemy until you finish, but often with weaker enemies, I would accidentally kill them between the time the prompt appeared and when I did the warp-strike.

I'm currently in chapter 8 and some good candidates for this are things like bulettes and shieldshears. There are hunts for each of these that can be redone at anytime, which will respawn the enemies:

Shieldshears: Longwythe Rest Area - Cranky Crustaceans
Bulettes: Cauthess Rest Area - Hammer the Cannibals
