I do believe and hope that this is all correct... trying to go off memory here:

This requires you to gain entry and enter the secret areas of each dungeon that is applicable to the trophy (not all you will find will have such).

The Nether Nebula
Near the cloister where the triad threshold was located, you'll need |SMASH| to break a boulder then |FLUTTER| to cross a gap.

The Watchplains
Passing where you fought the giant goblin, you'll come to a small encampment where you'll find a Black Chocochick.

Pyreglow Forest
Following the hint from one of the Occult Fan magazines, in the maze section of the forest requires you to follow the pyreflies to find the secret area. Here, you'll find a Magic Pot.

Icicle Ridge
The large ice pond that you must traverse after fighting Fenrir to get to Saronia has a blocked path you need to |SMASH|. Once up there, solve the weighted platform with a weight of 11+ and 100+ Earth Resistance to break the ice. Follow the new path northwest to get to a set of Holy Dragons.

Saronia Docks
Using the Occult Fan as a basis, you can find out that the trick to figuring this one will require three bells. The bells correspond to a set of graffiti in the slums of Saronia. Ring the bell near the kelp first, then to the shells, and finish up with the fish. By ringing the last bell, you'll summon the treasure ship with a Kupicaroon squad and several chests.

The Dragon Scar
This one is actually required to progress through the area; it is the location where you find Cerberus. (You'll find the item the Gimme Golem is asking for behind it.)

Valley Seven
In Cauldron 3, you'll find that you need |CHILL| to access a weighted lever. Bring several fire-resistant Mirages to reach the 150 resistance requirement. After dealing with the Murkrift and solving the platform's request, you'll come into Phoenix's domain.

The Windswept Mire
Once you reach the save point deep within the Mire, you'll also find a weighted platform. By unlocking this, you'll open the passage to the Flan Princess.

The Phantom Sands
In Toil 3, go forward in the middle and descend into the sand pit. Here you'll find a Sphinx and a treasure chest.

Underground Prison
Back where you separated from Squall but before meeting Shelke, bring a Mirage with |FLUTTER| to fly across the gap to reach a Mist Dragon.

Mako Reactor 0
First, defeat the murkrift on the same map as the timer-switch (that you use/used to get to the Reactor's core). This will give you the Gimme Golem's item. Reactivate the timer and loop around, but instead of heading towards the core, head back to the timer device and go to where the Golem is. You'll fight two large Cactuar mirages in this secret area.

Big Bridge
Whenever some Mythril Giants have been summoned, make sure you go out of your way to defeat them (even if they're on the opposite side of the bridge). Once you make it to the top and clear the last, the secret area will appear with an Iron Giant.

The Train Graveyard
Near where Vampire Prime is fought, you'll find a weighted platform. Successfully activating it will open the passage where four Murkrifts await. Clear these to summon a fifth with the mighty Nightmare mirage.

The Sunken Temple
Some progress into Chapter 21 will reveal an Intervention titled "The Sunken Temple's Secret". Completing this intervention will unlock the secret area with three chests inside.

The Crystal Tower
This one requires navigating a lot of the tower, taking down the Kuza behemoth mirages and claiming the Gimme Golem items from them. After you receive about three, return to the ground floor and use the stairs to head down, turning in your items as you come across the golems. At the end you'll find Titan.

The Chainroad
During the Ascent 5 map, head south to activate a warp to the secret area. At the end, you'll find King Paleberry.

Castle Exnine
During Postscript, return to where you fought the boss to find the secret area.
