  So after following the guide decapitating the worm and the end boss. I was still left without the trophy.
  
  After going through some forums I finally found the answer...
  
  You have to use a stun grenade on the tiny monsters, you know the really small ones that run at you and usually come in groups of 4 or 5.
  
  They come in the first wave of the sewer level so just equip your stun grenade and it will pop straight away if you've done the worm and end boss.

  There's just two enemies that are hard to decapitate, the worm and the boss - this video should help if you're having trouble with them!

  Seems glitched for me, decapitated them both for sure and got all of the rest naturally. Did you decapitate the worms on any other of the levels too?
