This is pretty straightforward - just hold down cn_L1 to heal yourself when you've taken damage. Repeat two more times and the trophy is yours.

I got this while on the way to meet up with Jacob. There are a lot of soldiers shooting at you and many opportunities to take damage. smile You'd almost certainly get it just playing through them game though.
