Maxing your tension is when the bar around the character portraits is flashing white/green. This is when you get the maximum amount of time when you Fairize. Do note that this trophy tracks the amount of times the tension is maxed, not when you utilize Fairize, so it will passively keep count for you and pop up during combat.
