To make the Bullet That Can Kill the Past, you have to find its four separate components and turn them in to the Blacksmith on the Fifth Chamber.

Important: You do not need to find all of these in one run. If you find one and turn it in, it'll carry over to all other runs.

The first component is the Prime Primer. It's the easiest of the four to get, as you just have to buy it from the shop. it's expensive, though, at 110 shells.

The second component is the Arcane Gunpowder. This can be found in a large room in the Third Chamber. You'll have to navigate across mine carts that rest on invisible tracks. Get in the close one and move in it until you line up with another, then dodge roll to that one. Keep going until you make it to the other end of the room. If you have a flying item, this is easy as pie, although it's really not hard without. As a note the Arcane Gunpowder is the only item of these four that is an Active item, so you'll have to replace whatever Active item you have to carry it. Your best bet might be using the Scout to get it, since he can carry two Active items. The map is the same every time.

The third component is the Planar Lead. It can be found at the end of a long room in the Fourth Chamber. There's an invisible path over the large gap in the room that you have to travel over to get to the Planar Lead. If you have a flying item, this is super-easy. If not, having a gun that leaves some manner of puddle on the floor (i.e. the Mega Douser or Plunger) will make things easier since the puddles will show where the invisible path is. In lieu of something like that, some guns drop casings behind them when they fire or reload. If you're incredibly patient and this is your only option, you can back up to the pit and fire away from it, and when the casings fall, they'll stay visible if there's a path behind you. As a side note, this room's path DOES change between runs, and will also change if you fall in the pit while trying to get across.

The final component is the Obsidian Shell Casing. It can be found after defeating the High Dragun at the end of the Fifth Chamber. Once you beat it, its skull will fall to the ground. Shoot it until it explodes, and there's the Obsidian Shell Casing.

Once you turn the last of these four items in to the Blacksmith, she'll craft the Bullet That Can Kill the Past, and the trophy will unlock.
