This isn't a particularly hard game to Platinum, but doing things in the right order will make your life a lot easier!! This is what worked for me.

First playthrough
Beat the game on Normal difficulty. On this playthrough, you should also:
- Complete all side quests
- Collect all collectibles
- Pick up the other random trophies (like The Triple Lindy)
Essentially, by the end of this first play through you should have all but 6 of the trophies (BFA, Apocalyptic and Deathinitive difficulty trophies, Wicked K and Crucible trophies, and Level 30 trophy).

New game plus playthrough
Start your New Game Plus on Deathinitive difficulty. Sometimes the game locks up if you try to start your NG+ straight away - as long as you've beaten the final battle, then restarting the game and continuing your last save will prompt you to begin an NG+. The difficulty select is at the very first screen (with the Gamma settings, etc).
- Don't touch the difficulty AT ALL
- Run through the game again. You can skip the side quests and collectibles and just stick to the main story. The Death Tombs can be useful for possessed weapons, however. You will get the trophy for hitting level 30 probably mid-way through.
- The boss battles are the toughest part. Ensure you have decent resistance, and a good set of possessed scythes with (ideally) health steal/health on crit, some sort of elemental damage, crit chance boost and crit damage boost.
- Once the Well of Souls becomes available (the final battle), DON'T GO THERE JUST YET. Make a USB backup of your save file. This might not be necessary, but it's a helpful backup - the original version of the game sometimes deleted your save upon completing NG+.
- Finish the Well of Souls, giving you the trophies for beating the two toughest difficulty levels.
- If you can reload your save prior to the final battle, do so. Otherwise restore your save from your USB backup and load that. At this point, DROP THE DIFFICULTY TO NORMAL. The Crucible scales to difficulty level - it's a breeze on Normal but pretty tough otherwise.
- Now you can go and complete the two Crucible trophies. Upon getting these two, the platinum trophy BFA will drop!
