You can get this playing any mode. Make it through the entire game without dying and this will unlock. The two story instances where you die (the mid-game boss fight against yourself, and an instance later on when you find the Athetos Clone) don't count against you and don't interrupt your progress.
Like many PS4 games, if you die, as long as you exit to the dashboard and close the application before it saves, you'll be fine. You'll lose any progress made up to that particular checkpoint, but your run will be intact. If that progress was not lost, that means you weren't quick enough.
It should unlock once you make it to the final stats page at the end of the game.
