This is a deviously hard trophy to get, and while I don't have it, I did want to offer a few pointers from my (failed) attempts.

This trophy requires you to save all cats and people while completing all of Miss Ions tasks in a single game. So far as I can tell, the glitch that allows you to gain several "complete the game with x" trophies by simply doing the last level doesn't work for this. You actually have to have what is essentially a perfect run.

1) Know your Miss Ion missions: There are 2 per level and they alternate with the Caretaker who is essentially a shop keep. You will, as far as I can tell, always get the missions in same order. For example, level 1-1 will always have "find the red bag" while 1-3 will always have the "find the five files" mission and so on. Much like how you have to learn how to plan your attempt on the level, you'll want to learn how to efficiently handle these missions. For instance, on level 1-3, you might as well simply walk around and search everything until you find your five files.

Of particular importance is the last one, which is a mission that can actually be failed. If you rescue the wrong person, you'll fail the mission, Miss Ion will yell at you, and won't follow you. Since this is the last mission in the game, make sure that you know how to deal with it. You definitely don't want to get through a perfect run and then fail on this.

2) Upgrades: Make sure that you've purchased all Stat upgrades, as these will help you quite a bit. Also make sure to visit the shopkeeper and get whatever you can. Most importantly, you really want to get the heartbeat monitor which will show people (and cats, I believe) on the map, as well as the defibrillator which allows you to revive people if they die.

Before entering a room, use the heartbeat monitor to see if there are people in the room, and find where they are. This will allow you to hopefully plan and counteract and stray fireballs by targeting the fires near them first. The defibrillator will help you if you fail. Note that the defibrillator does NOT work on Cats, so if you lose one of them, you're toast.

Also of note are the Super Hose and Speed Boots. The Speed Boots are an obvious advantage in a game with a time limit, but the Super Hose is exceptionally powerful. It's not something you need early on, but it makes getting rid of the green fires in the later levels far faster.

Note that if you can't get the first two early on, and hopefully the Super Hose by level 3, you could be in big trouble. If you don't have the money, you could always spray the Caretaker with your extinguisher and steal the items, but keep in mind that this changes the Grim Reaper to a Devil that is more dangerous.

3) Practice and stay calm: Fire kills those who panic. If you don't panic, you're fine. Unfortunately, the game is procedural, so there's an element of chance, and you could end up in a position that is, realistically, just not feasible. You might never get the chance to buy a defibrillator, not be able to know there's a person in a room, and before you find out, they are down. You're going to have to acknowledge that this may happen. This single trophy will likely take several hours and many frustrating attempts. It's going to be a real test of your patience and skill with the game.
