You'll see as BJ sits in a chair for quite a long time while recalling his vegetative state after the metal crashed into his skull at Deathshead's castle. Nazis come and go, frequently taking asylum patients for experiments.

Note: The walkthrough will detail the locations of both the health and armor upgrades. Upgrades carry over between timelines, so if you had some reason to play through chapters twice, you can at least benefit from that. The only way to have these upgrades reset is to start a new game or by manually deleting all save data outside of the game.

Waking Up

When the time comes for the asylum to be shut down, grab a knife from your food and wait for the one nazi in the room to approach you so you can take him down. Push open the door leading out of the room and crouch walk over to the throwing knife behind the Nazi facing away from you. Equip it and cn_RSc throw it at him to instantly take him down, then pick it back up and look to the right to throw it at another guy. If you want, you can do this five times total to get a new perk. Its successor comes with 20 more kills done in the same fashion.

Once these two guys are down, walk to the end of the hallway and try to stealthily take down (throwing knife or from behind) as many people as you can, particularly the commanders. Clear the floor once the threat of a broadcast has been eliminated. Take note that there are no enemies in this chapter from which you can knife-swipe helmets off of for armor.

Wyatt: Look for a bathroom with a bathtub in the middle of it. It'll be in the southeast corner of the starting floor. Inside, after a flashback, is a locked locker with an (1/1) Armor Upgrade inside. These increase the amount of armor you get per armor pickup by 10%, but the cap will always remain at 100.

Checking Out

You should soon lead to a staircase leading down, which is next to the metal bars barricading some white doors shut. Go up the staircase to find an (1/3) Enigma Code. A dead nurse by the stairs leading down has a shotgun and some ammo by her. If you're curious, you don't have the shrapnel mode unlocked for this thing yet, so you can't get its perk right now. Crouch walk down the stairs so you can have a chance of stealthily taking out a few guys downstairs, then look around for a different, smaller set of stairs leading back up to the upper floor (or take the original staircase back up to look for it in the southeast corner). There should be a crack in the stairs that gives you a small bit of room to shoot out the door, allowing you to reach (1/1) Izabela's Letter.

Return to the ground floor and approach the exit door to find that it's locked. Head to the north to find the button that unlocks it.

Fergus: There's a safe right behind the button to unlock the front door. Open it up and take the (1/1) Health Upgrade to increase your maximum base health by +10. This means that overcharge will not drain extra health until it surpasses 110 for this first upgrade. The maximum base health you can get is 200.

A horde of nazis already inside the building will appear to attack you once the door is open. Hide in the room you're in and shoot at them until it's safer to approach. Clean the place out and walk outside when you're ready to enter yet another firefight. Focus solely on killing Nazis and take cover inside the building as another vehicle comes in. It'll launch drones as one or two new soldiers leave it to shoot you. Using the shotgun to take care of the drones would be a good idea. Before reaching Anya, look under the front of the parked car to find a (1/1) Gold Hood Ornament.

BJ will take the parked car to leave the region with Anya, facing a few hurdles along the way. Anya will eventually take the wheel to head to her grandparents.
