To get your first quiz evaluation, you need to go to about six small asteroids in outer space. As far as I can tell, it doesn't matter what answers you give -- it seems like this game is aimed at kids to help them learn about themselves? whatever! -- so go to six and answer all the questions as quickly as you want.
Once you go to enough asteroids, there will be a phone with Dr. Therapist's picture in the lower-left corner of the screen. Tap L1, and it'll say you've answered enough questions to get an evaluation. You can either manually fly home or tap the shortcut button after opening the phone. Either way, once you get there, talk to Dr. Therapist and the trophy will ding.
